Cost of Care
We are an in-network provider for many insurers and accept all assigned Medicare patients. As plan coverages sometimes change, please verify that your insurance is accepted at the time you make an appointment.
Bring your current insurance card(s) to every visit. Insurance eligibility and demographic information will be verified at every visit; it is the patient’s responsibility to make sure that all billing information is up-to-date. If there is a change in coverage or your contact information, please let us know.
Although we will bill your insurance, full payment is the patient’s responsibility. We will make every effort to verify your coverage prior to your appointment, yet we cannot guarantee coverage of services by your insurance company. To determine your insurance benefits and eligibility, please contact your insurance carrier directly.
Some insurance plans require a referral to receive the services of a specialist, such as a cardiologist. These plans will not pay for services without a referral, and you may either reschedule your appointment or pay for the visit in full.
Beginning January 1, 2022, if you plan to pay for care yourself, or are uninsured, we are obligated to provide a “good faith estimate” of expected charges if you ask for one. The good faith estimate is not a bill. It includes expected charges for the primary service you are scheduled for and other items or services provided as part of the same scheduled service.

We accept cash, personal checks, money orders, and MasterCard or Visa. A returned check fee will be added to your account for every check returned for insufficient funds, stopped payment, or closed account.
Cash/money orders are only accepted at the main office (Anchorage) and Cath-lab. Cash is not accepted in Soldotna or Matsu.
Balance for Previous Services
Our billing specialists will discuss any balance on your account and assist you with making payment arrangements.
Collection
Unpaid balances will be referred to a collection agency if other arrangements have not been made.

This notice is in accordance with AS 18.23.400 disclosure and reporting of health care services, prices, and fee information. Effective January 1, 2020, and until further notice, Alaska Heart & Vascular Institute charges the undiscounted fees for the listed health care services provided as downloadable PDFs below. You will be provided an estimate of the anticipated charges for your non-emergency care upon request. Please do not hesitate to ask for information. Please note, a health care provider that provides a “good faith estimate” to a patient under (g) and (h) of Section 18.23.400 Disclosure and reporting of health care services, price, and fee information is not liable for damages or other relief if the estimate differs from the amount actually charged to the patient.
